Consent form for minors
For medical examinations and treatments for those under 18 years of age, parental consent is required.
Please print this out, fill in the required information, and bring it with you.
If you are accompanied by someone, you will be asked to fill out the form on the day of your visit.

5-minute walk from Yurakucho Station on the JR Yamanote Line and Keihin-Tohoku Line.
4-minute walk from Exit C1 of Ginza Station on the Tokyo Metro Marunouchi Line, Hibiya Line, and Ginza Line.
It is located right next to the historic Taimei Elementary School, just off Miyuki Street, a short walk from the Ginza Five exit.
Access from Ginza Station
① Head towards Exit C1 from inside the subway station.
② Exit through Exit C1.
③ When you exit through Exit C1 and turn around, you will see "GINZA5".
④ Enter "GINZA5".
⑤ Proceed straight down the corridor.
⑥ Continue straight along the path and exit through the entrance.
This is the view when I left.
⑦ Exit "GINZA5" and immediately turn left.
⑧ After walking a short distance, you will see a yellow sign for "Nippon Chozai Ginza Taimei Pharmacy," so enter the building entrance on the right.
⑨ Proceed down the corridor and you will find an elevator at the end.
⑩ The Tokyo Private Clinic is located on the 3rd floor.
